摘要

本文係探討一位聾啞初產婦的自然產產後照護護理經驗。此個案與案夫均為聾啞人士，與人的溝通靠著手語、唇語及筆談。個案在住院期間因無法用語言表達而造成產後照護溝通及學習上的障礙，護理期間為95年8月25日至95年8月28日，共計四天。經由筆談、觀察等方式收集資料，運用Gordon十一項功能性健康評估模式評估，發現個案有言詞溝通障礙、無效性母乳哺餵、照顧者角色緊張等護理問題。筆者在護理個案前，先以筆談的方式自我介紹，建立治療性關係及彼此溝通的模式，在護理過程中以帶動個案肢體的方式教導，並且以筆談的確認方式來了解個案的認知程度，使護理措施確實達到成效，解決個案的各項護理問題，出院前將行政院健康局出版的產後衛教本內頁所提供的網路資訊提供給個案，完成個案出院護理計劃。

This case report aims to present a nursing experience of a postpartum primipara with deaf ＆ dumb. This case and her husband are deaf and dumb persons, depend on sign language, lip reading and conversation by writing with people’s communication between 8/25/2006 to 8/28/2006. Nursing assessment was based on the framework of Gordon's functional health status. Data was collected by the conversation with writing, observation. Three nursing problems had been identified as follows: communication, impaired verbal; brestfeeding ineffective; caregiver role strain. Before attending to the case, the author introduce self by way of conversation in writing first, establishing the therapeutic relations and communicating with each other. The author instruct the case by way of gesturing and body language, and find out about the cognitive degree by way of affirmation of conversation in writing, enabling the nursing intervention to really to reach the effect. The case is advised on how to look after, and nursing baby. She is also fold to go online for other information before leave the hospital.
