一位動靜脈畸形併腦出血青少年術後恢復期之照護經驗

A Nursing Experience of an Adolescent Patient with Post-Operative Arteriovenous Malformation

摘要


本文為照護一位動靜脈畸形併腦出血術後恢復期之青少年患者,筆者於2012年4月3日至4月9日,藉由觀察、會談等技巧收集資料,運用羅氏適應模式進行護理評估,確立個案有身體心像紊亂、照顧者角色緊張及潛在危險性跌倒等護理問題。於照護期間,發現個案對於術後外觀樣貌有許多負向想法。利用主動關懷及接受等支持溝通技巧,重建個案正向之自我概念。此外,主要照顧者案母為待業中的單親媽媽,經濟負擔沉重,且本身視網膜病變需近期手術,自加護病房轉出時,也主動表示不知如何照顧,及需要社會資源協助,故主動聯繫各領域團隊人員,提供資源協助,並教導家屬照顧技巧,預防跌倒之發生,緩解照顧過程中之緊張及壓力,終使個案能順利返家休養。

The article described a nursing experience of a teenage patient with post-operative AVM. Observation, interviews and Roy's Adaptation Model were used to collect data from April 3rd to April 9th, 2012. The patient was confronted with body image disturbance, caregiver role strain and risk of falls. During the nursing process, the patient indicated negative thoughts on her postoperative appearance. Communication skills such as active care and acceptance were applied to assist the patient in building positive self-concept. In addition, the primary caregiver was a low-income single mother with diabetic retinopathy, financially burdened, and was in need of retinal surgery. The author provided social resource and care skills to relieve the caregiver's anxiety and stress, and to assist the patient to return to her daily life.
