摘要

好的紀錄能輔助醫療的進行。安寧共同照護（安寧共照）是由原診療團隊和安寧療護團隊共同參與，需溝通平臺作為資訊的聯繫，故發展「癌末安寧共同照護評估」紀錄表。表單信度、效度檢定完成後，用於某教學醫院52組接受安寧共照之癌末病人與家屬，平均使用期間為14.09日。資料以SPSS統計。結果為紀錄表包含「病人症狀困擾」48項、主要家屬之「照顧負荷」14項暨嚴重度與「醫療處置」40項三部分。紀錄表能呈現病人之症狀困擾、家屬的照顧負荷與嚴重度。共照期間醫療照護內容以非藥物性；末期問題協商與會談，各佔醫療處置總項目之45.73%；34.55%。本表能改善臨床醫療資訊的取得與溝通的便利，提升大眾對安寧緩和醫療照護的認知，具有推及非癌症末期病人照顧的參考價值。

Good records could benefit nursing care. Since hospice shared care involves the collaboration of two teams of medical specialist, a communication platform is required for information exchange to take place, and ＂terminal cancer hospice care assessment recording form＂ was designed for that particular purpose. Upon the confirmation of form validity and reliability, the form was applied to 52 pairs of hospice shared care cancer patients and families with an average duration of 14.09 days. The form included three main categories: 48 items on cancer patient ＂symptom stress＂, 14 items on family member ＂caregiver burden＂ and 40 items on ＂interventions of medical treatment＂. The form could demonstrate the symptom stress of the patient, caregiver burden of the family members and its severity. During the period of share care, the treatment was non-pharmacological and consisted of consultation on terminal issues and meetings, which occupied 45.73% and 34.55% respectively out of the entire medical management. The form could improve the acquirement of clinical medical information and the convenience of communication, enhance the public cognition of hospice care, and could serve as valuable reference for the care of non-terminal patients as well.
