照顧一位年輕女性接受放射線治療導致口腔合併症之護理經驗

A Nursing Experience of a Patient Diagnosed with Nasopharyngeal Carcinoma Suffering from Radiotherapy-Induced Oral Complication

摘要

本文描述一位年輕女性罹患鼻咽癌接受放射線治療，導致口腔合併症之護理經驗。筆者於2013年8月1日至8月16日，運用Gordon十一項健康功能評估為工具，透過直接護理、觀察、會談、身評及病歷查閱等方式進行資料收集，確認有放射線治療導致口腔合併症引起的疼痛、營養少於身體所需與無望感等健康問題。在照護期間結合跨團隊照護方式，調整飲食型態，改善口腔黏膜炎引起的疼痛。我們亦提供多樣化飲食方針，設計低目標進食量，讓個案逐步達成，藉此提升信心，增加自我控制感，並引導個案回顧生命之意義，重新拾獲求生目標並擬定生存計畫，使其能接受疾病帶來的衝擊並積極接受治療。

The article described a nursing experience of a patient diagnosed with nasopharyngeal carcinoma suffering from oral complication induced by radiotherapy. The nursing period was from August 1st to 16^(th), 2013. Through compressive evaluation, observation, physical assessment, consultation, and chart review, the health problems, established based on Gordon 11 functional health pattern, included oral complication induced by radiotherapy, malnutrition, and hopelessness. A model of interdisciplinary team was applied. During the nursing period, pain induced by oral mucositis was eased by adjusting dietary pattern. The calorie requirement was gradually met by offering various dietary instructions, and designing basic goals of food intake to build the patient’s confidence and self-control through gradual and progressive achievements. The author also conducted a guided life review and set up the survival plan to prepare the patient for the adverse impact that came with the disease and to actively accept treatment.
