腫瘤病房護理人員執行化學治療護理指導品質之改善方案

摘要

一套完善的化學治療護理指導可降低病息對未知事件的焦慮與害怕，也提高對治療的正向感受。本專案目的在（一）經由調查了解護理人員及病患對化學治療的認知與執行之現況，（二）進一步發展改善方案之成效探討。調查期間自91年3月15日至5月31日，調查內容包括「護理人員執行護理指導的完整性」及「病患對化學治療的認知」二部分。調查結果發現腫瘤病房護理人員執行化學治療護理指導的品質偏低，經分析確認主要的問題為：缺乏明確的化學治療護理指導標準與指引、缺乏完善的化學治療護理指導執行與評值規範、教育訓練不足及缺乏一套多樣化的護理指導輔助工具。經由舉辦執行化學治療護理指導之教育訓練，制定執行化學治療護理指導之標準與指引單、規劃一套完善的化學治療護理指導執行與評值規範，以及製作一套完整且多樣化的護理指導輔助工具等改善辦法，並對護理人員及病患作結果檢測。結果發現護理人員執行化學治療護理指導完整率由72%提昇至90%，病患對化學治療認知之正確率由75%提昇至86%，較專案改善前有明顯提高。本專案結果建立一套完善的化學治療護理指導規範，達到持續性品質改善的目標。

It is a completed nursing directive of chemotherapy care that releases patients' anxiety and worry about their treatment. The purpose of this project was to improve the quality of performance in nursing directive of chemotherapy care in oncology ward. The project was done by questionnaires from March 15, 2002 to May 31, 2002. It focused on the completeness of nursing directive of chemotherapy care and the patient's recognition to chemotherapy. Results of this study revealed that the quality of nursing directive of chemotherapy care was not good enough. Through further analyses, the main problems were unclear nursing directives, the defective tool of quality assessment, lack of training and lack of tool to supplement-assisted health education. Therefore, programs aimed to correct aforementioned shortcomings were initiated, including educational training, formularizing the standard procedure of nursing guide of chemotherapy, establishing a set of completed assessment tool of performance and evaluation, and designing a set of varieties of supplement-assisted health education. The results showed that the quality was much satisfied. The ratio of completed performance in nursing directive of chemotherapy care increased from 72% to 90% and the ratio of the patients' correct recognition to chemotherapy raised from 75% to 86%. This project succeeded to establish a completed set of nursing directive of chemotherapy and improve the quality of care.
