一位肺癌患者行肺葉切除術之手術全期護理經驗
Nursing Experience in Perioperative Nursing for a Lung Cancer Patient with Lobectomy
摘要
本文主要探討一位肺癌患者，經化學治療，行肺葉切除再化學治療之手術全期照護經驗。筆者於93年2月6日至93年2月20日期間，以Orem理論及手術全期護理爲架構，透過談訪、觀察及身體評估等收集資料。照護過程中的健康問題有術前：焦慮；術中：潛在高危險性神經肌肉血管及皮膚受損；術後：呼吸道清除功能障礙等。護理措施在術前爲提供支持及手術相關教育訊息，增加個案對手術的認知並減輕焦慮。術中爲給予個案神經肌肉血管及避免皮膚受損的完整保護，提供完全代償性的護理。術後爲教導有效性深呼吸咳嗽，增加肺活量及預防肺擴張不全，有效將痰咳出，維持呼吸道暢通恢復健康。建議未來確實應用手術全期護理，使接受手術之個案獲得身心靈的需求與滿足，提昇手術病患之護理品質。

This article probes into the perioperative nursing experience of a lung cancer patient initially treated by chemotherapy and later underwent lobectomy plus chemotherapy. The researcher used Orem theory to collect the materials through interview, observation and physical assessment methods during 93.2.6-93.2.20. Health problem prior to operation was anxiety. During operation the major nursing problems were potential damage of nerve tissue, muscle, blood vessels and skin. Recovery of respiratory tract functions was the major problem after surgery. Nursing measurements and relevant educational information prior to operation were provided to increase patient understanding of surgery and to lessen their anxiety. Total compensatory nursing care were provided during operation to avoid damage to nerve tissues, muscles, blood vessels and skins. After operation, patients were given instructions to cough the phlegm out effectively, to breath deeply for proper lung expansion and to maintain lung capacity. The authors propose that in the future, the theory of whole one operation could be applied thoroughly. By doing this, physical as well as psychological satisfaction of those undergoing operation will be met, nursing care quality can be promoted.
