摘要
胃癌在世界上統計癌症發生率為第五位，而死亡率為癌症第二位，估計2007年在美國有21,260人罹患胃癌，其中約有11,210人因胃癌而死亡，根據歐洲及美國統計五年存活率為10-20%，但是在stageⅠ五年存活率可以到60%而stageⅡ可以到34%。大部份的人發現胃癌時都在60至70歲左右。世界上在中國、南美洲、東歐及日本胃癌的發生率都蠻高，在台灣地區胃癌發生率也頗高，在所有癌症發生率中，以前它排名都在前五名以內，雖然胃癌的發生率在台灣近幾年有下降的趨勢，但是我們對於胃癌還是不能忽視。近年來科技的進步，一些診斷利器像胃鏡超音波及新穎電腦斷層均可幫助臨床醫師早期診斷早期治療，對於外科醫師來說，胃癌手術的進步及淋巴廓清術標準化漸漸趨於一致，無可諱言外科手術是最有效治療胃癌的方法，但是微創手術、腹腔鏡手術普及化對於外科醫師又是一個新的挑戰。對於胃癌病人來說化療藥物及放射線治療儀器進步使得病人存活率提高不啻是一大福音，再加上標靶治療的日新月異更使得中、晚期胃癌病人有更多選擇可以來治療癌症，本篇藉由文獻的探討使得我們對於胃癌的治療有一個更新的看見及了解。

Gastric cancer is the second leading cause of cancer-specific death and although the incidence is decreasing, it still ranked fifth among all cancers worldwide. About 60% of new cases of gastric cancer occur in eastern Asia. Five-year survival remains at 10-20%, but new strategies for prevention, early detection, and treatment may improve survival rates. Chemotherapy helps to prolong survival in cases of advanced disease, but surgical resection is the most effective treatment for curable gastric cancer. The development and refinement of endoscopy and image techniques over the past two decades have improved the ability to detect gastric cancer. This article provides an overview of updates in diagnosis, surgical treatment policies, chemotherapeutical regiments, adjuvant therapies and target therapies for advanced gastric cancer. We hope that it could provide update knowledge of gastric cancer for clinical staff, and improve the overall survival rate in advanced gastric cancer.
