照顧一位青春期生殖細胞癌患者的護理經驗
The Nursing Experience of an Adolescent Germ Cell Cancer Patient
摘要
本文在探討一位十八歲男性罹患生殖細胞癌的患者，在面臨生理上的不適、同儕分離、中斷課業學習及住院治療的調適過程。護理期間自2007年2月26日至2007年3月26日，筆者以羅氏適應模式評估健康問題，經由會談、觀察及理學檢查等方式，收集主、客觀資料，瞭解病患住院期間面臨的壓力，並確立個案的護理問題，包含：低效性呼吸型態、角色功能改變、社交隔離等護理問題。結合醫療團隊緩解個案呼吸困難的症狀，整合學校及社工等相關資源，運用同理心及溝通技巧，引導個案說出心中想法及感受，並重新調整對學生角色的期望及主動與他人互動，且持續給予病患心理支持及鼓勵，發揮護理人員照顧者、護理指導者、諮詢者的角色，協助病患面對疾病及青春期發展上面臨的壓力，也從中學習到癌症對青春期病患心理社會層面發展的影響。期望本篇文章能提供護理同仁在照顧此類病患時之經驗分享，提昇護理照護品質。

This article discusses an 18-year-old male patient suffering from germ cell cancer, confronted with adaptation process of physical discomfort, peer separation, academic learning interruption and hospitalization. The author adapted Roy Adaptation Model of Nursing to evaluate his health problems through interviews, observation and physical assessment etc., in order to collect subjective and objective information of the pressure during hospitalization, and to establish the nursing problems during the period of February 26, to March 26, 2007. The problems found are: ineffective breathing pattern; change the role and function; social isolation, etc. The author integrated medical team to help relieve the patient's symptoms of respiratory difficulty; and also integrated related resources from his school and the social work department, using empathy and communication skills to guide the patient to speak his minds and feelings; readjusted the expectations of the role of students and active interaction with others, continuing to provide psychological support and encouragement to the patients. To bring the nurses, nursing instructors and advisory rolls into full play, the author assisted the patient coping with his illness and adolescent development pressures. In the meantime, the author gained knowledge of the cancer effects on the adolescent's psychosocial development. The author hopes to share her experience with the nursing staff and enhance the quality of nursing care of this kind in future reference.
