運用疲倦照護指引於非何杰金氏淋巴癌患者接受化學治療期間之照護經驗

Nursing Experience of Using Practice Guidelines for Cancer Related Fatigue in a Patient with Non-Hodgkin Lymphoma during Chemotherapy

摘要


本文為運用疲倦照護指引於一位首次經歷嚴重疲倦之非何杰金氏淋巴癌患者之照護經驗。護理期間(2011年2月13日至2011年3月2日)以美國國家癌症資訊網(National Comprehensive Cancer Network, NCCN)提出之臨床照護指引評估患者的疲倦情形,透過觀察、會談、病歷查閱等方式收集各項資料,發現個案因情緒困擾(焦慮)、營養狀況改變少於身體所需而導致疲倦護理問題。筆者於照護期間與個案建立良好的治療性人際關係,針對個案之生理(營養)及心理(焦慮)需求予以協助,並教導個案癌症相關疲倦之知識,降低個案之生理不適及焦慮感,進而有效的改善個案疲倦程度。期望此照護經驗可提供臨床護理人員更加瞭解癌症患者在治療過程中疲倦症狀的評估方式及照護需求。

This case report describes nursing experience by using practice guidelines of fatigue assessment and management in a patient with non-Hodgkin Lymphoma experiencing severe fatigue during chemotherapy. The patient received chemotherapy for autologous peripheral blood stem cell harvest. The clinical practice guideline for cancer related fatigue published by National Comprehensive Cancer Network (NCCN) was used to assess patient's fatigue level and pattern between February 21 and March 2 in 2011. Data was collected by observation, interview, chart review. Emotional distress and nutritional deficits were two major factors that may lead to the patient's fatigue. The author established a good therapeutic relationship with the patient, offered assistance based on his physical and emotional needs, and educated the patient how to manage fatigue. The author did not only improve the level of fatigue of patient, but also alleviate the physical discomfort and anxiety successfully. This case report provides information to help nurses understand how to assess and mange fatigue in taking care of patients who received chemotherapy.
