運用Orem自我照顧理論協助一位腹膜透析患者的護理經驗

A Nursing Experience of Using Orem Self-care Theory to Help a Patient with Peritoneal Dialysis

摘要

腹膜透析是一種長期透析治療方式，若未妥善處理，將會使生命受到威脅。本文個案為65歲之老年人，因末期腎病必須接受透析治療，筆者於腹膜透析前、後，運用Orem自我照顧理論為評估架構，評估個案自我照顧能力及所需要介入的護理系統，確立個案有末期腎病之病程與治療的知識不足、對操作腹膜透析換液技術的知識不足及營養狀況改變少於身體所需等三個急需處理的護理問題。在整個護理過桯中，筆者針對個案的問題藉由部分代償及支持–教育性護理系統的介入，提供合宜的護理活動，不斷鼓勵個案，進而協助個案完成自我照顧。

This article described the application of Orem’s self-care theory in guiding the nursing care for a 65 year old client with end stage renal disease (ESRD). Three self-care deficits were addressed including lack of knowledge on disease progression, management of therapeutic CAPD regimen and nutrition requirements for people with ESRD. The partial compensatory and supportive education nursing intervention was implemented to help the patient understand the nature of his disease, effectively manage the therapeutic regimen, and adapt the life changes caused by this illness.
