摘要

此篇論文之目的係在探討心智遲緩與唐氏症候群之基本概念、定義與其疾病發生率與流行率在公共衛生與社會福利政策之重大含意。不論在第一級預防，第二級預防與第三級預防，都需要有一套有效之治療與預防方法，來減少心智遲緩與唐氏症候群之發生率與流行率。
第一部份討論心智遲緩之定義與分類，根據國際疾病分類與美國精神疾病診斷與統計手冊第四版，均依智商程度分為輕度、中度、重度及深度四級，而美國心智遲緩協會最近出版之分類系統卻依據心智遲緩者所需要之支持程度(Levels of Support)以達最佳之功能(Optimal Functioning)來界定心智遲緩之程度，例如：需要間歇性支持、有限度支持、擴張性支持以及普及性支持四級等。
第二部份討論心智遲緩之發生率與流行率，比較開發國家與未開發國家中，心智遲緩流行率之特性。並指出低度社會經濟身份為心智遲緩最強烈與最常見之預測因素，尤其是輕度之心智遲緩。但較嚴重之心智遲緩發生率與流行率則常與遺傳因子相關連。因此，在預防保健上，對於輕度與重度之心智遲緩，醫療衛生與社會福利政策將各有不同之指標。
第三部份分析心智遲緩之顯著原因，由染色體異常、畸形發展到唐氏症之產生，由染色體之減數分裂與有絲分裂期間之不分離狀態，到三體性染色體之變化引起唐氏症，還有產前、產後或圍產期之感染與外傷之環境因素所引起之異常。
第四部份討論唐氏症候群在心智遲緩中之遺傳因素及其嚴重特性，說明唐氏症之危因與產婦之年齡呈正相關。唐氏症之先天性身體結構之缺陷與生存期間愈長，其所伴隨之疾病愈多，對於整個醫療照顧與社區服務以及其家庭支持，將形成巨大的負擔，
第五部份及第六部份討論心智遲緩之預防工作與社區服務，需要從最基本之基因遺傳協談、預防感染到一般與特殊之社區服務，著手落實預防成果，甚至於推動國際合作，並要尊重智障者之生存權利與人權保護。
於結論中建議公共衛生、社會福利與教育輔導以及啟智安養機構，還有智障者家屬，必須共同合作，提出有效之預防與治療之公共政策，督促政府有關機構研擬可實行之預防、治療與復健政策。

This paper is to present the basic concepts and definitions of Mental Retardation and Down Syndrome from the perspectives of public health and preventive medicine. The classification of mental retardation was briefly discussed in regard to the classifications of International Classification of Diseases and Diagnostic and Statistical Manual (DSM-IV).
The incidence and prevalence of mental retardation are discussed between the developed and developing countries. Their characteristics are also presented in this paper. The Epidemiologic characteristics of severe and mild mental retardation are compared. The severe nature of Down Syndrome in mental retardation and its prevention as well as community services to prevent mental retardation are also discussed.
