障礙者個別化服務計畫格式之建構

身心障礙者是否能接受過性的服務，主要以個別化服務計畫為依歸，一份好的服務計畫將有效管理相關專業服務的品質。為了協助教保員改善現行個別化服務計畫執行上面臨的困境，本研究透過量化與質化並行的方式，為由問卷調查及焦點團體等方法，研訂個別化服務計畫的標準格式。問卷調查在97年7-9月間進行，共計發出400份問卷，回收361份，回收率達90%，另依據問卷調查回覆結果，針對有意願接受本研究深入訪談者，優先邀請成為焦點團體對象。焦點團體在97年10月間辦理，分成北區、桃竹苗區、中區、南區等四區，每場次10人，共計40人，出席人數計有27人。研究結果顯示有高達九成以上的受訪者同意個別化服務計畫的標準格式應包括六部分，即「諮詢建擋」、「評估」、「計畫」、「執行」、「年度總結評量」、「轉銜／轉介」而「諮詢建檔」包括「初步諮詢」、「面談建檔」、「個案初評」；「評估」包括「分項評估」、「評估結果綜合分析」；「計畫」為「擬定服務計畫」「執行」為「執行紀錄」「年度總結評量」為「綜合評量與建議」「轉銜／轉介」為「資源轉介／轉銜」。未來建議可在此通用架構下微幅增加不同障別特殊需要的項目，增進其完整性與可應用性。

The service for disable person is suitable or not, it will depend on the individualize service plan. The service plan which can manage the professional team quality effective is good. So that, this study will help to find out how the individualize service plan should be. This research is through quantization and quality way to run side by side, by methods such as questionnaire investigation and focus group. Questionnaire investigation went on during July-September of 2008, sent out 400 questionnaires altogether, retrieved 361, the rate of recovery reached 90%. Group handled the focus during October of 2008, is divided into four districts such as north district, north district 2, center district, south district, every session is 10 people, it is 40 people altogether, attendance has 27. Result of study reveal have, up to 90% more than interviewee agree, melt standard form planned to serve should include six parts individually: consultation and establish the file, assessing, plan, carry out, year measurement, transition/transfer. Future is proposed to develop different form editions, and to suit for different disable people.
