Chylous Ascites after a Bilateral Nephrectomy in a Diabetic Patient: Case Report
一位糖尿病患經雙側腎臟切除後發生乳糜性腹水：病例報告
摘要
手術後乳糜性腹水是後腹腔手術罕見的併發症，其發生於一位糖尿病患因氣腫性腎盂腎炎經雙側腎臟切除後，則更為罕見。我們報告一位40歲的女性糖尿病患因上述原因，發生手術後乳糜性腹水；經由腹水引流、中鏈三酸甘油酯為主的飲食，並經全腸道營養、體制素（sandostatin）注射等保守療法，而成功治癒。

Postoperative chylous ascites is a rare complication of retroperitoneal surgery. Chylous ascites is even rarer after a bilateral nephrectomy due to emphysematous pyelonephritis in a diabetic patient. We report a case of a 40-year-old diabetic female, who suffered from chylous ascites following a bilateral nephrectomy for emphysematous pyelonephritis. The patient was successfully treated with therapeutic ascites drainage, a medium-chain triglyceride (MCT)-based diet, followed by total parenteral nutrition (TPN) and sandostatin administration.
