先天性心臟病青少年自我調節運動方案之設計

Designing a Self-regulated Program of Exercise for Adolescents with Congenital Heart Disease

背景和目的:隨著醫學精進先天性心臟病童已可存活至青少年甚至成年以後,實證顯示大多數手術後的患者心肺功能和運動能力可達到一般人的程度,然而先天性心臟病青少年的運動量及運動強度卻不若預期,因此,本文之目的即為設計先天性心臟病青少年運動介入方案。方法:通過倫理審查後於北區兩家醫學中心之兒童心臟科門診,招募符合條件之個案共六位參與活動,以自我調節理論中自我監測、自我評價及自我回應三個概念為核心,輔以心臟病患者教育計畫的五個步驟為策略,設計為期四週的介入方案,配合台灣青少年之生活作息特色,於每週日舉行一次90分鐘的討論。結果:先天性心臟病青少年運動自我調節方案是一個可行而有效的運動介入措施,能協助青少年從興趣中去選擇適宜的運動類型,調整運動強度,建立規律的運動習慣。結論:本方案為一先驅研究,並在測試方案之可行性以及過程評值方面提出建議,期盼未來能進一步的以實驗性研究測試方案之成效。

Background and purpose: Due to medical advances, most children with congenital heart disease (CHD) are expected to survive to adolescence and even to adulthood. Evidence indicates that most of their cardiopulmonary function and exercise capacity can reach the level of that of normal peers after early intervention; however, adolescents with CHD are often engaged in inadequate patterns and intensities of exercise. The aims of this study were to design a self-regulated exercise program which met the characteristics of development and limitations of illness for adolescents with CHD. Method: After obtaining approval from the Institutional Review Board, we recruited six participants from the outpatient cardiology departments of two medical centers in Taiwan. We utilized the concepts of self-monitoring, self-evaluation, and self-reaction as the core elements of the self-regulated program and developed program activities based on them. Four 90-minute sessions were held on Sunday mornings during the summer vacation of 2011. Results: Preliminary data showed that the constructs of self-regulation theory could operate empirically, promote adolescents with CHD to choose adequate patterns of exercise based on their own interests, and further adjust the intensity of their exercise. Conclusion: Suggestions for evaluating a program of self-regulation were proposed. Future studies should be conducted to evaluate the effectiveness of a self-regulated program of exercise for adolescents with CHD.
