代理護理長角色壓力之經驗

Role Stress among Acting Head Nurses

摘要


目的:本研究旨為探討代理護理長產生之角色壓力及其影響因素。方法:以立意取樣收案某精神科教學醫院11位代理護理長,於93年4月至10月以半結構方式深入個別訪談與團體座談方式收集資料,採質性研究法進行資料內容分析。結果:研究結果發現研究對象多重工作角色身份,身兼臨床照護和行政管理業務;而經歷角色過荷、角色混淆、角色衝突等角色壓力。歸納影響角色壓力之因素包括自我期許過高、同儕認同不足、主管支持性低、以及角色任務不明,導致自己與他人對其不同角色的期待有所差異。結論:因此明確規範代理護理長工作職責,開放雙向溝通與給予支持,強化其知識和經驗並進,提供行政管理實務訓練,以提昇其核心能力與工作滿足,使其面對角色轉換過程,得以勝任新的角色。

Background and Purpose: The research study adopted a qualitative approach to explore how a group of acting head nurses experienced role stress and what they felt about this. Methods: The study included eleven subjects who were chosen from a psychiatric center by purposive sampling. The dataset was collected by semi-structured interview between April 2004 and October 2004. The records were then analyzed by content analysis. Result: The results show that acting head nurses play multiple roles and take on the duties of nursing care and nursing administration simultaneously. They experience overload, role ambiguity and role conflict. Stress factors include over self expectation, poor identification from peers, lack of support from managers and unclear task identification and these induce a gap between the subjects and other staff. Conclusion: The findings suggest that it is important to establish for the individual a clearly defined role, which exists in a creative and supportive environment. Furthermore, they should be offered nursing administration training that emphasizes knowledge related to practice. This will increase their core competences and job satisfaction during the role change process.
