Circumcision is one of the most commonly performed operations worldwide and it rarely results in complications. However, the incidence of complications varies from 0.2 to 38 percent in different series. Circumcision is usually performed for religious and medical reasons. Buried penis resulting from fatty mons pubis in obese men sometimes mimics phimosis. Circumcision in these people may be misdiagnosed by an inexperienced urologist. Inappropriate circumcision may result in more penis shaft skin defect and more serious penis retraction known as concealed penis. Here we present the case of a 168 Kg obese man with buried penis who received inappropriate circumcision, which resulted in severe penis retraction with difficult urination. The glans was trapped by the tight preputial orifice, which consisted of a circular granulomatous scar, and could not be exposed. We used supra-pubic incision procedures and three-stage scrotal tubal flap for reconstruction. The result was satisfactory at two months' follow-up.
