不同訓練方式對長照機構服務員感控認知之成效

Effectiveness of Different Training Methods on Improving the Knowledge of Infection Control among Nurse Assistants in Long-Term Care Units

摘要


目的︰本研究探討不同教育訓練方式對提升長期照護機構照顧服務員感染控制認知之成效,以提供機構管理者於規劃相關教育訓練之依據。方法:針對臺北市19家長期照護機構之166名照顧服務員,以立意取樣方式並依研究比例分為實驗組84人及對照組82人,運用「長期照護機構照顧服務員感染控制認知評量表」為評量工具,前測2週後再作後測,兩組之差別在於是否實施教育訓練介入。結果︰實施不同方式的訓練措施後,兩組照服員對於『認識感染源』的認知因機構評鑑的等級不同而有顯著差異,且機構類別對其『正確洗手』方面的認知亦有影響。實驗組在實施教育訓練介入後的認知成效較對照組具顯著差異(p<0.05)之外,其評量結果亦較介入前提升,其中以「教育程度」最具影響力(p<0.01)。結論︰教育訓練介入可提升長期照護機構照顧服務員的感染控制認知且維護機構住民的安全。實施教育訓練介入時應考量學員年齡、學歷及工作年資因素的影響,並對其認知較差的部份作個別性及教學內容上的調整;此外,可運用雙向回饋機制及加強視覺提示的方式提昇教育訓練成效;增加評估一個月、六個月及一年後之認知,並定期實施相關教育訓練以強化因時間而淡忘的認知。期望本研究的結果及教育訓練方案可提昇長期照護機構照顧服務員之感染控制認知、提昇照護品質;並提供機構管理者作為感染管制課程規劃之依據。

Object ive: This research explored the effectiveness of different training methods in improving the knowledge of infection control among nurse assistants in long-term care units. The findings should be valuable for planning of educational training. Method: One hundred and sixty-six nurse assistants in 19 long-term care units in Taipei were purposively sampled and divided into two groups: 84 people in experimental group and 82 people in control group, differed in the presence of educational training. The "Assessment for Effectiveness of Training Programs upon Infection Control Knowledge for Nurse Assistants in Long-term Care Unit" was applied to test the change of knowledge for infection control before and after the two-week training. Results: There was a significant difference (p<0.05) in identification of infection sources in facilities with different hospital accreditation. There was also significant difference (p<0.01) in the practice of hand washing among those working in facilities of different hospital accreditation. In addition, the educational training intervention had remarkable effects in experimental group, especially for those with lower educational level (p<0.01). The subjects in experimental group had improved test results after the educational training, and also higher effectiveness in the improvement of knowledge of infection control as compared to the control group. Conclusion: The intervention of educational training can effectively improve the knowledge of infection control among nurse assistants in long-term care units and enhance patient safety. Age, educational level and seniority of the nurse assistants should be taken into consideration in educational training, and training material should be adjusted to enhance the weakness of each individuals. In addition, bidirectional feedback and visual hints can be gradually applied to elevate the efficacy of educational training. Long-term evaluation at one month, six month and one year and related educational trainings can be conducted to keep the memory fresh. We expect this research and associated educational training projects can improve the knowledge of infection control among nurse assistants in long-term care units, which can further be applied in clinical work and improve the quality of the units. The findings of this research also provide useful information to the administrators of long-term care units in planning infection control training courses for nurse assistants.
