摘要

104 年起台北市立聯合醫院推行社區安寧照護，藥師首度參與團隊服務。本文以文獻探討配合實務經驗方式說明藥師在安寧團隊中所擔任的角色，討論：(1) 藥師應如何提供藥事服務及(2) 分享執行過程實際經驗。藥師於安寧療護團隊所扮演的角色包括評估處方、提供建議、監測療效與提供相關服務。結果發現經由執行下列各點可提昇藥事服務價值：善用科技如雲端藥歷系統；導入居家與長期照護經驗；提供以病人為中心之藥事服務；給予管制藥品法規諮詢等。所得結論是藥師應密切與團隊溝通合作，服務更可由「專業用藥評估」延伸至「團隊陪伴」。藥師應持續增加安寧照護相關知識與技能，與安寧療護團隊合作積極管理藥物治療問題，以保障病人權益。

The Taipei City Hospital has contributed to community palliative care service since 2015. From then on, pharmacists of the hospital started to participate in palliative care and offer home-visiting hospice pharmaceutical care. This article describes the role of pharmacists in palliative care team by referring to literatures and actual applications. The purposes of this article are as follows: (1) to explain how pharmacists implement pharmaceutical services in palliative care and (2) to share our experiences in optimizing supportive palliative care. The role of pharmacists in palliative care team consists of evaluating therapy, recommending, monitoring therapeutic efficacy, and performing other medication-related functions. We found high quality multi-disciplinary palliative care requires both traditional and contemporary pharmacist skills. To achieve this goal, pharmacists should use modern high-tech products like PharmaCloud System, introduce the approach in home care and long term care system services, provide pharmaceutical services which are concentrating on the patients, and offer consultations about controlled drugs to solve medication-related problems. In conclusion, pharmacists should collaborate closely and actively with the team members on the therapies and treatments of the patients. Gradually, pharmacists can expand the pharmaceutical services from merely medical therapy evaluation to caring of patients with the palliative care team, entirely. Moreover, pharmacists should develop correct knowledge, attitudes, and skills on palliative pharmaceutical care, and collaborate with the whole team on the management of pharmacotherapy to protect the patients' right.
