Sysmex XE-2100 Alpha儀器評估

Nasal Leech Infestation Masquerading as Unilateral Subacute Rhinosinusitis

摘要


本研究以CLSI的H20-A規範評估兩台XE-2100血液全自動分析儀的正確性、精確性、相關性並驗證正常參考值,以及以流式細胞儀為標準法,探究該儀器的光學與阻抗兩種方法其血小板結果的正確性。白血球的Swt,Srr與Sdd分別為2.6%、4.7%、5.6%,紅血球為0.9%、2.4%、2.3%,血紅素為0.8%、2.9%、1.9%,血小板為3.7%、8.3%、8.7%。再就SE-9000與XE-2100相關性而論,WBC、RBC、HGB、HCT、MCV與PLT的相關係數r^2為0.99-0.98,至於NEUT%,LYM%,MONO%,EO%,BA%的r^2各為0.97、0.97、0.74-0.67、0.95 與0.36-0.32。白血球五項分類的精確性與相關性皆符合廠商的試劑允收標準。以手工為金標準,兩台XE-2100 其報告的敏感性、特異性、偽陰性、偽陽性與一致性分別為97.2%、95.5-93.2%、2.8%、4.5-6.8%與96.3-95.0%。此外驗證並接受廠商所提供的正常參考值。當檢體的紅血球及血小板均正常時,光學法(optic)或阻抗法(impedance)與被認為金標準的流式細胞儀比較,其相關性極佳(O: r^2=0.9900; I: r^2=0.9702);若檢體紅血球正常,不論是否出現巨血小板,光學法及阻抗法對流式細胞儀的相關性差異不大(O: r^2=0.9919, I: r^2=09766);然而當出現紅血球型態異常但不論是否出現巨血小板,對血小板數量少於150×10^3/uL的檢體,光學法相較於阻抗法有明顯的改善(O: r^2=0.9687, I: r^2=0.8930),這些紅血球形態異常包含MCV<80fL或RDW-SD>50f L亦或RDW-CV>16%。本評估結果認為XE-2100分析儀的正確性、精確性、相關性皆符合評估要求,並接受廠商所提供的正常參考值。且證明紅血球的形態異常比巨血小板更易干擾阻抗法,因此光學法優於阻抗法。

Rhinosinusitis is a common disease in otorhinolaryngologic clinics. It is diagnosed and treated by chief complaints and physical examinations. A 19-year-old female patient presented with left rhinorrhea, sneezing, itchy throat, cough and left frontal headache for over 1 month. Unilateral subacute rhinosinusitis was impressed, and medicine failed to cure her symptoms at other clinics. However, unilateral nasal leech infestation was identified at our clinic. After the nasal cavity was locally anesthetized, the worm was removed under endoscopy. Herein the rare case is reported as a warning that we should take more time to evaluate a nasal lesion, and a nasal leech infestation will not be misdiagnosed as unilateral rhinosinusitis.
