摘要

目前我國長期照護之機構數及照護人力足以滿足老年人口之需求，然而相關的研究卻指出長期照護機構由於政府政策法令不明；地方主管執行能力不彰，專業權責區分不易，再加上個案執行資源分配不均，品質評估標準不明確等諸多因素，因而衍伸出同業間削價競爭的現象，不但照護品質無法提昇，更使得長期照護機構之管理經營陷入重重困境。因此，如何掌握機構內現有之資源，降低作業成本，提高經營效率，並以提昇病患醫療照護品質為手段，延續照護病患之生命，就成為長期照護機構管理者，現階段經營管理上之重要議題。
為能使衛生主管機關，能夠順利建立全國性長期照護中心病患資料庫，建置長期照護資訊系統，並成為未來病患資料上傳之資訊平台。因此，藉由文獻蒐集、整理、研究與實地訪談的方式，並佐以政府對於長照機構發展之政策，歸納長期照護管理資訊系統所應具備之功能及介面，建置一套完整的長照機構管理資訊系統。個案長照機構對本系統評價甚高，未來可利用網際網路將資訊提供給合作醫院，提高照護品質與效率，亦可讓家屬在家中即可了解親人狀況。

At present, the demand of bed number and manpower are far beyond the supply in the long-term care (LTC) service in Taiwan. Numerous researches indicated that due to the government's unclear policy, incapability, vague responsibility, uneven allocation of resource in each case, the LTC institution management become more and more difficult and fall into a heavy predicament.
How to control the resource allocation, reduce the cost, improve the efficiency, promote the quality, and prolong the life of patients are important issues of LTC management. Applying information technology to establish a LTC management information system can definitely help the situation. This can start from the department of health setting up a nationwide LTC patient database, as a platform for all LTC organization.
This study reviewed literature, consulted with the executive of practitioners and government officer of LTC, and summarized the current needs of LTC institution in Taiwan to set up a suitable LTC information system. The LTC information system was then adopted by a case to evaluate the performance of the system. The evaluation results were reasonable. Future research can use the Internet to share the patient information to hospital and family members to provide better care.
