本專案目的在改善安寧病房護理人員執行出院準備之困難並提昇其出院準備完整率。本單位曾於2004年進行出院準備之改善方案,於改善後執行率雖有提昇。但此次現況分析發現,出院準備的品質並無持續的提昇,因而引發改善動機。專案自2006年8月至2007年5月,經由(1)強化護理人員的認知,提供在職教育及制定作業流程。(2)提昇護理人員出院準備執行能力,解決其執行之困難。(3)提昇出院準備執行完整率,鼓勵護理人員一起參與表單的修改。結果顯示(1)提昇護理人員知識由57.5%提昇至100%。(2)提昇出院準備執行率由48%提昇至100%。本專案也列入年度計劃,每季評核一次,期望能達到品質的持續提昇。

The purpose of this project was to solve the discharge problems for nursing staff and to improve the discharge planning service. An action research was conducted from August 2006 to May 2007. To identify the reason for palliative care nurses who can not complete the discharge planning, a semi-structured questionnaire was developed by the research team to survey palliative care nurses. There was 57.5% nurses lack of discharge planning knowledge and 45% nurses report that the equipment and supplies management has not standardization. Most of nurses complain that the discharge planning record is ineffective. The project first enhanced nurses' discharge planning knowledge and skill by team discussion and discharge planning training course. The research team established and audited the discharge planning standard and practice to conform procedure. The discharge planning records were modified by the nurses themselves. After 9 months implementing this project, the results show that the nurses' discharge planning knowledge was increase from 57.5% to 100%. Furthermore, all of nurses were pay more attention to solve the discharge problems and can complete the discharge planning document (48% vs. 100%). These findings may provide as a useful reference in clinical practice for palliative care ward. Further research is needed to evaluate the satisfaction of discharge patients and their families.
