研究目的:探討臨床護理人員執行「不施予心肺復甦術」(Do Not Resuscitate,DNR)之現況及相關因素。材料與方法:本研究為橫斷式相關性研究設計,以南部某區域教學醫院的臨床護理人員為研究對象。以結構式問卷為研究工具,共發出問卷314份,回收有效問卷250份。所收集之資料除描述性統計外,另進行向前選取之多元羅吉斯迴歸分析。結果:發現年資較深、年齡較長、在急重症單位服務、曾上過安寧課程、死亡課程及其親人有DNR經歷的護理人員,有較多執行DNR的實務經驗。結論:DNR的執行是一項非常實務的臨床經驗,護理人員在現今的醫療環境下雖較處於輔助的角色,仍可期待藉個案照護之經驗分享、角色演練或跨團隊運作的多元教育訓練方式,來提昇臨床護理人員執行DNR的能力。
Research Objective: The purpose of this study was to investigate the current status and the related factors associated with the implementation of medical decision toward ”Do Not Resuscitate (DNR)” among clinical nurses of a regional teaching hospital. Material and Method: This study adopted a cross-sectional correlation study design. The study instrument was a structured questionnaire. A total of 314 questionnaires were distributed, and 250 effective questionnaires returned. The collected data were analyzed by the descriptive statistics and multiple logistic regression analysis with forward selection. Results: The results revealed that those nurses who were more years of work experience, elder, served at emergency room and at intensive care unit, ever hospice educated, ever death educated, and experienced relative's DNR were more likely to the execution of DNR. Conclusion: Although nurses are more in the auxiliary role in health care environment, the implementation of DNR is a practical nursing experience, via multi-disciplinary education programs, such as sharing experience of case nursing, role rehearsal, and cross-team working approach would promote clinical nurses the capability of executing DNR.
