兒童心智門診提供父母諮詢之現況探討-以新北市為例

A Study of Parent Consultation in Child Psychiatry Clinics in New Taipei City

摘要


目的:研究探討台北縣市教學醫院中,設有兒童心智門診之精神醫療單位提供父母諮詢服務之現況,包括父母諮詢的種類、內涵、以及臨床專業人員實施父母諮詢時的環境與現實因素。方法:本研究採問卷調查法,首先以深入訪談法、紮根理論與修訂式德懷術進行問卷編製,再對台北縣市教學醫院設有兒童心智門診之醫療單位的醫師、心理師、護理人員與社工人員進行問卷調查與分析。結果:研究歸納出專業人員在個別父母諮詢中提供10種諮詢模式,47項諮詢內涵;團體父母諮詢中包括7種模式、17項內涵;演講與座談會形式父母諮詢中,包括7種模式、13項內涵;研究者同時列出7項專業人員推動父母諮詢時常見的困難。結論:調查顯示父母諮詢是臨床人員相當重視的服務項目之一,提供父母諮詢服務的比例超過樣本九成,但普遍缺乏固定的時間結構與系統化的介入方案。

Objective: The study aimed to investigate the current clinical practice for parent consultation, including different models and contents. It also intended to explore elements that hinder the administration of parent consultation in child psychiatry clinics in Taipei.Methods: In the first stage of the study, a ”Parent Consultation Service Questionnaire” was established based on in-depth interviews with 12 child psychiatry experts from major clinics in northern Taiwan. Grounded theory and modified Delphi method were employed to modify the questionnaire. In the second stage, a survey was conducted on 47 child psychiatry professionals, in which the Parent Consultation Service Questionnaires were administrated in order to obtain data regarding the current status of parent consultation.Results: The results of qualitative analysis showed that various parent consultation models and contents were found in individual parent consultation, group settings, and public speech and parent workshops. Seven elements that hinder the administration of parent consultation were reported.Conclusion: The result concludes that parent consultation has becoming an important clinical service in child psychiatry clinics. However, there is a lack of systemized theoretical approaches as well as support from the medical system to enhance the services of parent consultation. Limitations were discussed, and suggestions were made for improving the serves of parent consultation in child psychiatry clinics.
