血管收縮/擴張反應、D型/非D型人格的高血壓患者在生氣情境下之自主神經系統反應

The Autonomic Nervous Responses in Hypertensive Patients With Vasoconstriction/Vasodilation Reactivity and Type D/Non-Type D Personality During the Anger Recall Task

摘要


目的:探討高血壓患者在生氣情境,血管收縮組/擴張組合併D型人格之心臟自主神經活動。方法:共納入72位高血壓患者分為血管收縮組/擴張組以及是否合併D型人格,所有參與者皆經歷生氣作業並蒐集其自主神經指標。結果:67%參與者在生氣情境出現血管收縮反應,33%參與者在生氣情境出現血管擴張反應。血管收縮組是否合併D型人格者之自主神經反應無明顯差異;在血管擴張組,D型人格之整體心跳變異與副交感神經活動顯著高於非D型人格者。結論:在血管擴張合併D型人格組,副交感神經活動撤除、整體自主神經活性較高,推論其交感神經活動無明顯撤除;在血管擴張未合併D型人格組,末梢血管在基準期緊縮,生氣描述時擴張,整體自主神經活性明顯低下。

Objective: The purpose of this study is to examine the relationships between peripheral vascular responses with type D personality traits and autonomic nervous responses during the anger-induced task. Methods: Seventy-two adults with elevated blood pressure were recruited. The participants were then divided into the following two groups by anger description reactivity in blood volume amplitude (BVA; %) including vasoconstriction group and vasodilation group. And, the participants were then divided into the following two groups including type D personality and non-type D personality. BVA and heart rate variability (HRV) indices were collected during the following five stages of the anger recall task. Results: Sixty seven percent of the participants demonstrated a typical vasoconstriction reaction during the anger description stage. The remaining 33% of the participants demonstrated a vasodilation reaction during the anger description stage. During anger recall task, the autonomic nervous activity with or without type D personality. During baseline and anger recall task, the BVA of vasodilation group was significantly lower than that of the vasoconstriction group. In non-type D personality, the total HRV reactivity indexed by the standard deviation of normal to normal intervals (SDNN) and parasympathetic activity indexed by HF of the vasodilation group was significantly lower than that of the vasoconstriction group. In the vasodilation group, the total HRV reactivity indexed by SDNN and parasympathetic activity indexed by HF of type D personality was significantly greater than that of the non-type D personality. Conclusion: The results of this study demonstrated vasoconstriction and vasodilation reaction during the anger description stage reaction. The parasympathetic nervous activity, total HRV in the vasodilation group with type D personality were higher than the vasodilation group without type D personality. The results of this study demonstrated that the vasodilation group with type D personality may evoke more autonomic nervous system activity to dilating. The vasodilation group without type D personality may not evoke autonomic nervous system activity to dilating.
