以癌症醫療照護團隊觀點探討健康醫療、緩和療護與長期照護之整合

Integration of Health Care, Palliative Care and Long-Term Care: From the Prospective of Cancer Care Team

摘要


隨著強調滿足需求與以病人為中心的文化變革,健康照護、緩和療護與長期照護的整合成為當前健康照護體系變革的重要議題。因為連結整合各類照護不但可避免服務提供的零散與重疊,增加資源使用效率,也能提升服務輸送品質。對於受照護者來說,可以得到更好的整合性與全面性照護。以癌症病人而言,基於相關法律規範,目前已發展癌症多專科醫療照護團隊與安寧緩和醫療團隊提供健康照護與緩和療護。然而,癌症病人健康照護、緩和療護與長期照護的整合模式仍未完整建立。因此,本文目的在於探討健康照護、緩和療護與長期照護之關係,並修正Lynn和Adamson於2003年提出的軌跡模式,對於癌症不同照護間之整合現況與未來展望也提出分析與討論。本研究期使能促進癌症病人有周全性、可近性與連續性的全人照護。

Regarding the emphasis on met need and patient-centered, the integration of health care, palliative care, and long-term care is a critical issue in the restructuring of the current healthcare system. Integration of the various care services not only prevents the fragmentation and overlapping of service provisions, and increases the efficiency of resource usage, but also improves the quality of the care delivered. For those in need, better integrated and comprehensive care can be achieved. Based on the relevant laws and regulations, a multidisciplinary cancer health care team and hospice care team can constructively contribute to providing health care and palliative care for cancer patients. However, the integration of health care, palliative care and long-term care for cancer patients is not yet well-established. Thus, this study aims to discuss the relationship between health care, palliative care, and long-term care, and to modify the trajectory model proposed by Lynn and Adamson in 2003. Furthermore, according to the analysis of the current healthcare system operations, proposals and suggestions are made for the further development of care integration. This study aims to make an effort to facilitate comprehensive, accessible, and continuous holistic care for cancer patients.
