灼燙傷病人出院後身體生活功能恢復程度與自我效能之相關因素研究

摘要

研究目的為瞭解灼燙傷病人出院後身體生活功能恢復的相關因素，以及自我效能是否能預測其恢復的程度。研究工具以結構式自我照顧信心量表評估自我效能及巴氏量表測量身體生活功能。本研究採橫斷式調查中部灼燙傷病人與其照顧者共97人。結果發現目前身體生活功能平均得分為88.83較出院時62.18高。出院時身體生活功能有64.9%輕度依賴與22.3%高度依賴均需給予協助，父母／本人、教育程度、居家環境的影響最為明顯(R^2=46.7%)；而目前身體生活功能則是受到父母／本人、受傷部位、受傷多久的影響(R^2=28.1%)。居家期間自我照顧信心平均5.61分（量表1~7分），得分依序是醫療資訊、家庭社會適應、社會資源、居家照顧、經濟來源等照顧信心，並可預測解釋44.8%(R^2)目前身體生活功能恢復的程度。因此，瞭解灼燙傷病人身體生活功能的恢復源自於自我照顧信心，尤其是建立在醫療資訊信心，建議應規劃灼燙傷復健之衛教指導與醫療訊息提供。

The purpose of this study was to identify factors related to recovery of physical function and self-efficacy among burns patients after discharge, and to establish whether self-efficacy is a predictor of recovery level. The Self-Care Scale Confidence and Barthel Index were used to collect data. A total of 97 subjects (burns patients and their caregivers) were enrolled in this study. The result showed that patients' average physical function score was 88.83, a higher average score than their 62.18 average upon being discharged. When they were discharged, 64.9% of them were mildly dependent and 22.3% highly dependent. all needing help. The factors influencing physical function upon discharge were parents or the patients themselves, educational background, and living environment (R^2=46.7%). The factors influencing physical function at the time of the study, however, were parents or the patients themselves, the injured part(s) of the body, and time required for healing (R^2=28.1%). During the home rehabilitation period, the self-care confidence scores were 5.61 (scale range: 1-7). This result predicts and explains levels of physical function recovery among 44.8% (R^2) of burns patients. An understanding of recovery of physical function among burns patients is therefore rooted in self-care confidence and, particularly, in confidence in information about treatment. We would suggest that, in relation to burns recovery, health education and guidance be prepared and treatment information provided.
