帕金森氏病患者之憂鬱傾向及影響因素探討

The Inclination to Depressive Mood and Related Factors among Patients with Parkinson's Disease

摘要


本研究目的在瞭解帕金森氏病患者之憂鬱狀態及其相關因素,採用橫斷式相關性研究設計,以立意取樣方式於北部某醫學中心,選取100位帕金森氏病患者;採皮爾森積差相關、獨立樣本t檢定、卡方檢定、單因子變異數分析、逐步複迴歸分析與邏輯迴歸分析等統計方法。研究結果發現:帕金森氏病患中:⑴有44%有憂鬱傾向;⑵罹病期間、帕金森氏病的症狀、病程分級、每日服用L-dopa劑量、睡眠品質、疲憊感與憂鬱呈顯著正相關,但日常生活功能與憂鬱呈顯著負相關;⑶疲憊感、帕金森氏病的症狀、睡眠品質是憂鬱的重要預測因子,可解釋總變異量的58.0%;⑷有疲憊感者產生憂鬱傾向的機率是無疲憊感者的3.76倍。此研究結果將可提供臨床醫護人員,在提昇帕金森氏病患者照護品質之參考。

The purpose of this study was to explore the inclination toward depression, and related factors, among Parkinson's Disease patients. A descriptive and correlation study design was used. Using purposive sampling, 100 subjects were recruited from a medical center in northern Taiwan. Data were analyzed using the Pearson product-moment correlation, independent t-test, chi-square test, one way ANOVA, multiple regression and logistic regression. The results showed that: (1) About 44% of Parkinson's Disease patients had depressive inclinations. (2) The duration of disease, total UPDRS score, Hoehen and Yahr stage, daily L-dopa dose (mg), quality of sleep, fatigue and depression symptoms were positively correlated; the Schwab and England score was negatively correlated with depression symptoms. (3) Fatigue, total UPDRS score, and quality of sleep explain 58.0% of the variance in depression among subjects. (4) Fatigue was a significant predictor of depression inclination (OR=3.76, 95% CI=1.44-9.82) among Parkinson's Disease patients. We may apply these results to promote health care quality among Parkinson's Disease patients.
