經尿道攝護腺切除個案術後疼痛及其相關因素探討

摘要

本研究探討經尿道攝護腺切除術個案術後的疼痛程度及其相關因素，研究採描述性相關研究法，以問卷調查方式收集資料。研究結果顯示80位個案，平均於手術結束後3小時開始疼痛，術後以第一階段（約術後15小時內）疼痛程度最高，其最嚴重疼痛程度平均為5.98分（0-10分疼痛數字量表）。個案術後第一階段疼痛程度與其過去手術經驗疼痛程度、攝護腺原大小、術前焦慮程度、術後使用止痛劑劑量有顯著正相關，而不同手術醫師之個案的疼痛程度亦有顯著差異；第二階段疼痛程度與術後使用止痛劑劑量、留置尿管大小有顯著正相關，不同手術醫師之個案的疼痛程度亦有顯著差異；第三階段疼痛程度與年齡、負向疼痛信念得分、手術時間、術後使用止痛劑劑量、留置尿管大小有顯著正相關。本研究之結論為疼痛的確受許多因素所影響，在疼痛照護上，除瞭解個案不同的疼痛程度、性質外，應瞭解影響個案疼痛之相關因素，如本次研究與個案疼痛有相關的焦慮、疼痛信念、尿管大小、年齡、手術時間、攝護腺大小等，而依個案不同的狀況予降低焦慮、增進正向疼痛信念、於疼痛開始時即給藥、在手術後可使用的尿管中選擇較小號的尿管等，以提供個案合適之照護，及減輕個案術後疼痛程度至最低。

This study explored the pain experiences, and the factors related thereto, of patients receiving transurethral resection of prostate (TURP). A cross-section relation design was used, with questionnaires to collect the data. The number of participants was eighty. They experienced pain at three hours after surgery on average. The highest level of pain intensity occurred during the first fifteen hours after surgery. It was 5.98 on average (0-10 scale). The results of the study were that the pain intensity was significantly positively related to age, the weight of the prostate, the patients' anxiety level, negative pain attitude, the size of the foley catheter, the duration of the surgery, and the use of analgesic dosage after surgery. The postoperative pain has multiple effect factors. In order to relieve patients' pain, we must understand the reasons behind that pain.
