Oral Health in Children and Adolescents Aged 6-18 years with Congenital Heart Diseases

六到十八歲先天性心臟病患口腔衛生調查

摘要


口腔衛生狀況不佳的先天性心臟病的小朋友接受牙科治療時,可能因暫時性菌血症而引發感染性心內膜炎。本篇主要目的是調查台灣6-18歲先天性心臟病小朋友的口腔衛生狀況。本實驗蒐集台大醫院兒童心臟科共137位患者,而所使用的問卷是依據美國兒童牙科醫學會的蛀牙危險評估表所製作,資料分析是利用商用統計算。在先天性心臟病的6-12歲的小朋友中,各組別平均deft分別為4.23,6.18,4.94,3.17,3.42,0.93和1.45。在13-17歲的先天性心臟病小朋友的DMFT分別為3.38,3.5,4.4,4.57和8.17。患有先天性心臟病的小朋友中有54.7%有輕微到中等程度的牙菌斑堆積,44.5%的小朋友有嚴重程度的牙菌斑堆積。而7-12歲小朋友的deft和defs都比2005年國民健康局所做的調查來的高。台灣先天性心臟病的小朋友口腔衛生狀況比一般小朋友的口腔衛生來得差。

The congenital heart disease (CHD) child with poor oral hygiene receives dental treatment may increase risk of bacteremia that lead to infective endocarditis. The purpose of this study was to investigate the oral health of children aged 6-18 years with congenital heart disease in Taiwan. A total of 137 children and adolescents participated in this study, selected from Pediatric Cardiology of the National Taiwan University Hospital. The questionnaire used in this study was the modification of the Caries Risk Assessment Tool of American Academy of Pediatric Dentistry. The data were analyzed by commercial statistic software. For CHD patients, the mean deft indices of each age group from 6 to 12-year-old children were 4.23, 6.18, 4.94, 3.17, 3.42, 0.93 and 1.45, respectively. The mean DMFT indices of age groups from 13 to 17-yearold adolescents were 3.38, 3.5, 4.4, 4.57 and 8.17. Thin or moderate dental plaque accumulation was found 54.7% of the patients and heavy plaque retention was found in 44.5% of the patients. The deft and defs index of age groups 7 to 12-yearold were higher than that of the survey published by Department of Health in 2005 Taiwan. In Taiwan, the oral health of the CHD children and adolescents was poor as compared with the general pediatric population.
