某區域教學醫院精神科急性病房病患超長住院影響因素之分析
An Analysis on the Related Factors of Overstay Hospitalization for Psychiatric Patients in the Acute Ward of a Community Hospital
摘要
本研究旨在分析精神科急性病房病患超長住院之病患人口學、主要照顧者及疾病醫療照護等因素。研究設計採病例對照研究法，以回溯方式登錄病歷資料以進行資料之收集。研究對象爲某區域教學醫院精神科急性病房民國92年1月1日至92年12月31日出院之病人共269人，依住院天數42日爲切點，分爲超長住院158人及非超長住院111人兩組，以邏輯式回歸統計方法加以分析。研究結果顯示：「無工作」、「有負性症狀」及「過去住院次數」等三變項與精神科急性病房病患超長住院有顯著相關；即無工作者比有工作者超長住院的機率高出2.50倍，有負性症狀者比無負性症狀者超長住院的機率高出2.20倍，過去住院次數每增加一次者之超長住院的機率多出1.13倍。針對超長住院之高危險群病患，護理人員應結合職能治療師與社工師共同參與提供病患結構式活動或工作訓練，並提供家屬如何應用社區精神復健資源訊息，同時住院期間應積極邀請家屬共同討論個案出院後之安置問題。

The purpose of this study was to identify factors of overstay hospitalization in acute ward of psychiatric patients. A retrospective case control study design was used to recruit the psychiatric patients of a community teaching hospital from January 1, 2003 to December 31, 2003. Overstay was defined as a hospital stay of over 42 days. There were 158 overstaying psychiatric patients enrolled and another 111 psychiatric patients without overstaying records were enrolled as control group. Research data were collected from the medical records and analyzed with multiple logistic regression. The result showed that unemployment negative symptom and past hospitalization frequencies are highly related to overstay hospitalization; the jobless has 2.50 times of higher chance than the patients who had job those who have negative symptom are 2.20 times as much as those without symptom, the chance increases 1.13 times of every past hospitalization. It is recommended that nurses should cooperate with occupational therapists and social workers and providing training programs of working ability to patients. It is essential also to provide their caregivers with psychiatric rehabilitation service information and encourage them to use it.
