摘要

本文旨在敘述照顧一位肺癌末期病人於住院期間面對生理不適、心理衝擊及靈性困擾等問題之護理經驗。護理期間由2015年1月23日至2015年2月13日，確立主要護理問題有低效性呼吸型態、哀傷及心靈困擾。護理過程運用安寧療護的理念，主動提供個別性、及時性之護理措施，緩解呼吸困難，經由傾聽和陪伴以了解病人在疾病過程所面臨心理衝擊，協助完成道謝、道歉、道愛、道別過程，並運用靈性照護技巧，協助病人獲得生理、心理、靈性三方面安適。建議護理人員應自我不斷學習，增加心理諮商及溝通的技巧，提升自己對瀕死病人整體性照護能力，落實安寧療護不應設限於安寧病房之限制，在有限的時間裡，協助病人渡過人生的最後一段路。

This article describes the nursing experience of a patient with end stage lung cancer to cope with her physical discomfort, psychological impact, and spiritual burden. Patient-centered care was provided from January 23^(th), 2015 to February 13^(th), 2015. The author provided hospice care individually to relieve patients' dyspnea and improve physical comfort, and tried to listen and understand her psychological impact of the disease in order to help her expressing gratitude, remorse, love, and farewell to whom she took to heart. Nursing staff should support end-stage patients to ensure their final days be meaningful and dignified and continuously study and improve psychological counseling and communication skills for dying patients on their final journey.
