從《傷寒雜病論》探討精神科用藥後錐體外症狀

Discussion on extrapyramidal symptoms caused by psychiatric medications from the Shanghan Zabing Lun

摘要

西醫使用的抗精神病藥物主要作用於腦部，以調控多巴胺為主要藥理機轉，但也因此常引發錐體外症狀（extrapyramidal symptoms），使患者出現動作和心理上異常的副作用。精神病在中醫屬於「癲」、「狂」，是陰陽過盛或不足的結果，而錐體外症狀，則可歸屬為「煩」、「躁」的症狀表現。《傷寒雜病論》中有數條與「煩、」、「躁」以及精神、動作異常相關的條文，筆者將嘗試探討這些條文與錐體外症狀的關聯性。

Antipsychotic drugs, which control dopamine concentration in brains, are mainly prescribed by psychiatrists to treat mental illnesses. However, extrapyramidal symptoms are antipsychotic drugs' main side effect, which causes movement abnormality and mental distress. According to Chinese medicine theory, mental illnesses are caused by excessiveness or insufficiency of yin and yang, and the book written by Zhang Zhongjing-Treatise on Exogenous Febrile Disease-has description on mental and movement abnormalities. In this article, I will discuss the relationship between the prescriptions in Treatise on Exogenous Febrile Disease and extrapyramidal symptoms.
