中醫治療泛酸心悸胸悶之病例報告

The Therapeutic Experience of Gastroesophageal Reflux Disease with Traditional Chinese Medicine - A Case Report

摘要

這位57歲女性，近兩個月有感胃梗、胸悶、泛酸、胸口灼熱等症狀但未曾就醫，本次因三週內發生三次自覺心跳停止暈倒而至西醫就診，西醫檢查心電圖無異常，胃鏡顯示Gastroesophageal reflux disease（GERD）, Grade A. Gastric erosions；患者夜間眠差不易入睡，多夢，易怒，大便硬一二日一行，小便可，經中醫辨證為肝膽氣逆犯胃、中焦痰熱濕阻造成脾胃升降不利而泛酸胸悶、痰火擾心而心悸，經治以疏肝清熱、化痰降逆，主方開立柴胡龍骨牡蠣湯及溫膽湯加減，用藥三個月後患者未再發作心悸暈倒且泛酸胸悶等狀況大幅改善達到治療效果。

A 57-year-old woman experienced heartburn, chest tightness and stomach upset about two months. Moreover, she suffered from severe dizziness and palpitation for three times in three weeks but no abnormalities in the electrocardiogram and in the blood test were noted. Owing to heartburn, gastroesophageal reflux and gastric erosions was observed under gastroscopy. She also had sleeping disturbance, irritability and defecation difficulty. Based on Chinese medical differential diagnosis, we prescribed Wen Dan Tang combined with Chai Hu Jia Long Gu Mu Zuo Tang. The gastric discomfort, acid regurgitation, and associated symptoms improved dramatically after approximately one month traditional Chinese medicine treatment. The traditional Chinese medicine treatment persisted two month and the gastric symptoms and dizziness did not relapse in this period. In conclusion, traditional Chinese medicine might be the complementary therapy of gastroesophageal reflux disease.
