衛生署於95年起,針對機構外具有多重問題之精神病患,依據個案管理為架構,提供即時與持續性專人訪視之社區關懷照顧服務,進而減少社區精神病患重覆住院。本研究目的為:(一)瞭解社區精神病人之需求;(二)瞭解病人接受社區關懷照顧服務之現況。以台北縣12鄉鎮市,160位社區精神病人為對象,採行動研究法並依據病人需求及社區關懷照顧服務情形,進行內容分析,研究結果發現:(一)社區精神病人以精神醫療需求最高;改善病人與家人關係及加強其生活自理能力次之。(二)社區關懷服務之內涵可歸納為:促進病人持續接受藥物治療、協助病人獲得家人支持、提昇病人生活的意義及提供整合性醫療資源等四大類服務。此結果可作為精神衛生政策推動及精神病患社區照護規劃之參考。
Case management has been used as a strategy by the Department of Health to provide continuing community care services for psychiatric patients since 2006 in order to reduce the readmission of those psychiatric patients in the community. The purposes of this study are: 1. To explore the needs of psychiatric patients in the community. 2. To examine the services offered for the patients through the continuing community care services. The participants are 160 patients from 12 townships and cities within Taipei County. Action research method was adopted in this study. The results demonstrated that: 1. The most important issue was the needs of psychiatric medical service. 2. Following was improving the relationships between patients and their family members and improving patients' self-care abilities. The continuing community care services offered for the psychiatric patients can be classified into four categories: improving the medication treatment compliance; obtaining family supports; promoting the meanings of life for patients, and linking health care networks. The results could be used as a reference for guiding the mental health policy regulation and health care services for the psychiatric patients in the community.
