日間病房反覆自傷之憂鬱症患者的護理經驗

Nursing Experience for a Day Care Depressive Patient Performing Repetitive Self-Mutilation

摘要


本文描述在日間病房照顧一位反覆出現自傷行為之憂鬱症個案的護理經驗。筆者自2008年2月1日開始照護個案,發現個案2006年11月8日住院迄今,無效性因應能力及自傷行為實質改善情形不佳,故除了平時的復健與團體護理治療之外,於2008年2月11日至4月14日介入十次個別治療性會談,與個案討論日記,會談中運用認知行為改變技巧,以「認知不良功能紀錄表」為輔,引導個案覺察自我情緒感受、辨識認知扭曲模式和修正自傷行為,訓練個案養成正向思考和行為的習慣。結果顯示個案在生活人際互動中,正向思考增加、自傷行為出現的頻率和強度逐漸降低、消失,壓力調適能力提升,並且重拾自我信心,故將此護理經驗提出分享。

This paper describes an experience of nursing a depressive patient performing repetitive self-mutilation while hospitalized in a day care ward. The nursing period started on February 1st, 2008, and the author discovered that the patient, who had been hospitalized since November 8th, 2006, had experienced ineffective improvement for cognitive distortions which had resulted in negative emotions and acts of self-mutilation. From February 11th to April 14th 2008, therefore, in addition to regular rehabilitation and group therapy, the author conducted ten individual therapeutic interviews. During these interviews, the author discussed the patient's diary with her and applied cognitive behavioral therapy, using the Dysfunctional Thought Record, not only to guide the case toward emotional self-awareness, identify cognitive distortions, and correct her self-mutilation behavior, but also to inculcate in the patient positive thinking as a means of facing problems. After the nursing intervention and cognitive therapy, the patient's positive thinking in daily life and interpersonal interactions progressively increased, and the frequency and intensity of her self-mutilation gradually diminished, and even disappeared. In addition, the patient regained self-confidence and her coping skills improved. The author therefore considers this nursing experience worthy of sharing with the nursing profession.
