協助一位再次不足月早期破水經產婦的護理經驗

摘要

本篇個案報告是協助一位經產婦再次經歷不足月早期破水所引發生理、心理相關護理問題的過程。筆者籍由非結構式的會談、觀察、傾聽的方式，並以羅氏適應模式為評估指引收集資料，護理期間95年4月10日至18日，返家後電訪共5次，並將與個案之互動以12篇行為過程的記錄方式加以記錄、整理，經歸納主要護理問題有1.焦慮；2.潛在危險性感染；3.低效性母乳哺餵。在護理過程中，筆者採漸進、鼓勵的方式，以同理心、傾聽、陪伴、不批判態度、討論及會談過程記錄收集資料等技巧並運用相關資源，協助個案調適再次不足月早期破水之身體不適，克服心理的困頓，正向面對母親的角色。希望籍此護理經驗的分享，能提供護理同仁照顧類似病患有更多的了解，進而提供個案以家庭為中心所需之照護，使其成為稱職的母親。

This report is to describe the nursing experience of a multipara with recurrent preterm premature rupture of membranes, in term of patient physical and psychological aspects. Information was gathered by non-structural interview, observation, and listening. In addition, we also used the Roy's adaptation model to analyse the data collected. The nursing period was between April 10th, 2006 to April 18th, 2006. Another 5 telephone interviews were carried out after the patient being discharged. 12 sessions of behavior process were analyzed in total and the major nursing issues found included (1) anxiety, (2) risk for infection, and (3) ineffective feeding. Process of the nursing care included step by step and encouragement with empathy, listening, accompany, discussion and interview with non-judgmental attitude that helped the patient go through her psychological difficulties and physical discomfort so that she can take the maternal role positively. By sharing this nursing care experience, we can have a better understanding of these patients. By using a family-centered nursing care, we may help them play their maternal role well.
