一位腦中風患者及其主要照顧者的護理經驗

本文係描述一位65歲女性，因自發性腦中風引發右側偏癱的護理經驗，照護期間自96年7月23日至96年7月31日，筆者運用Gordon十一項功能性健康型態評估工具收集資料，發現以下之護理問題：身體活動功能障礙、自我照顧能力缺失、照顧者角色緊張等。筆者運用主動關懷、陪伴等方式與個案及其主要照顧者建立良好的護病關係，透過協助個案參與復健活動，直接指導個案技巧上的缺失，並協助個案有效的使用輔具，增加其自我照顧能力；並透過傾聽、介紹實際案例分享，鼓勵案夫說出內心的感受與困難點，降低主要照顧者之無力感及衝擊；筆者更應用淺顯易懂的衛教方式及口訣，在有限的時間內，增加個案及主要照顧者的記憶。經由護理此個案，筆者深刻的體會到護理人員不單只是照顧患者，更須涵蓋照顧者，使病患及照顧者能達到正向的適應。

This article described nursing experience of caring a 65-years-old female, who suffered from ischemic stroke in a result of right side hemiparalysis during July 23, 2007 to July 31, 2007.We use Gordon 11 functional health patterns assessment Tool to collect data and found the following issues about: Mobility about impaired physical function、self-care deficit、Caregiver role confusion. We built up good relationship between every cases and their caregivers by the way to care and accompany them in advance. To attend rehabilitation activities via assistance in every cases, We directed them the deficiencies of skills, and helped them with using auxiliary in order to improve her sefe care skills. We also encouraged patients to talk about the feelings and difficulties and reduced the powerless sensation of main caregiver by way of listening and sharing about actual cases. Furthermore, for a limited time, We used some easy understanding of pithy formulas to improve patients and caregiver's memory. During caring this case, we had realized that nursing should be not only caring the patients but also caring the caregiver which can also help them to adapt to a positive way.
