戒癮更生人之追蹤輔導流程訂定:質性研究

Designing a Counseling Program for Prison-Released Drug Offenders: Qualitative Method

背景及目的:2006年,地方紛紛成立毒品危害防制中心,但再犯率仍非常高。毒危中心的個管師多為約聘,流動率高,偏重個別傳承經驗。本研究對成癮者、個管師及志工老師(合稱輔導員)進行質性訪談,瀏覽逐字稿、編碼、賦予意義及內容分析,以訂定追輔新式流程。結果:共完成8位戒癮者、2位個管師與3位志工老師。歸納出追輔四階段:(1)預備階段:毒危中心接獲通報後,與個案連繫進行初步評估。(2)核心輔導階段:消除疑慮(輔導員應遵守倫理、避免官方色彩、緊守崗位、視個案如親)、瞭解與接納(提供自尊、訊息、工具性、社會性陪伴支持)、判斷(輔導員有敏銳的警覺、介入時機、持續追輔)。(3)復歸社會準備階段:包括穩定未復發及疑似再犯的兩種情形。(4)終結階段:2年內未再用藥列管期滿,或再犯入監。建議:採追輔新式流程,並評估成效;改善個管師工作權益,以降低離職率;多元資源整合。

Background: In 2006, the Drug Abuse Prevention Center (DAPC) administration changed from being under the central Ministry of Justice to the local level. However, the recidivism rate was still high. Most case managers in DAPC are contract-based, resulting in a high proportion of turnover. How the case mangers do the counseling for the prison-released drug offenders is by passing their experience one to one. Purpose: The aim of this study was to design a counseling program of prison-released drug offenders using a qualitative method. Drug offenders, counselers (include case mangers, and teachers volunteer) were interviewed by semi-structure guidelines. Original statements were coded to a descriptive or interpretative code, and combined into items. Results: Eight drug offenders, 2 case managers and 3 teacher volunteer were interviewed. Four stages in the program were suggested. (1) In the preparatory stage, counselers start to know the drug offenders after they release from prisons, including their problems of finance, living, family, diseases, and drug use. (2) In the core stage, counselers should first reduce the suspicion (by complying with the ethic, avoiding bureaucracy, not giving up, and treating offenders as family members), get the acceptance from the offenders (by providing support of esteem, information, instrument, and social companion), and alert any tendency of the drug-relapse. (3) In the society re-entry stage, continue giving social support to those who were stable and without relapse. Find out the problem of the relapse and adjust their own attitude for those with relapse. (4) In the closing stage, terminate the counseling services for those without relapse in 2 years after released from prison, or for those who relapse and return to prisons. Suggestion: We hoped that our counseling program is helpful for counselors to provide aftercare service. Effectiveness of our counseling program should be evaluated. The working benefits of the case managers in the DAPC should promote in order to reduce turnover rate. Multiple resources for prison-released drug offenders should be integrated.
