運用「多功能子母褲」於婦產科病人改善術後急性疼痛之方案

術後止痛是病人最重要的需求，但婦產科病人因須密切觀察出血狀況及會陰沖洗，需打開束腹帶，造成體位改變時傷口缺乏適當固定而牽扯的劇烈疼痛；調查發現病人疼痛0至10分整體感受為4.61分，會陰沖洗時疼痛感受為4.04分，對現行紙褲及束腹帶整體滿意度（滿分4分）低，僅有2.95分；進一步分析，因常須被解開束腹帶執行護理活動而減少持續固定傷口，是不滿意的主因，特組成專案小組決定以創新設計護理用品「多功能子母褲」來改善問題。量產推行後，病人的疼痛整體感受降為3.83分，且於會陰沖洗時仍能持續固定傷口，疼痛感受降為3.04分，整體滿意度提昇達3.63分；多功能子母褲透過專案研發量產，提升了術後病人急性疼痛照護品質及滿意度，並已推行至其他科腹部手術患者使用。

Pain relief is the most important need of post-operative patients. An abdominal binder is used to help fix incisions in obstetric and gynecologic patients in order to reduce pain. However, incision pain often increases when the binder and underpants are removed to observe the wound and irrigate the perineum. With the use of commercial binders and disposable paper underpants, patients' pain score (on a 0-to-10 scale) was a mean of 4.61, and a mean of 4.04 during irrigation of the perineum without continuous wound fixation by the binder. The mean integrated satisfaction score for the off-the-shelf binder and disposable underpants was 2.95 (maximum possible, 4). Our team designed a therapeutic medical binder incorporating underpants for use after lower abdominal operations, which allows perineal irrigation while maintaining continuous wound fixation. Patients who used this new binder reported a mean overall pain score of 3.83, with a mean of 3.04 during irrigation with continuous wound fixation. The mean integrated satisfaction score was 3.63. The newly designed therapeutic medical binder improved postoperative pain relief in patients after lower abdominal operations.
