照顧一位行輸尿管碎石術病人及主要照護者之手術全期照護經驗
Nursing Experience in Perioperative Nursing for a Patient Undergoing Ureter Lithotripsy and Main Caregiver
摘要
本報告是探討照護一位輸尿管結石行輸尿管碎石術病人，及其主要照護者病人執行手術期間之照護經驗。護理期間自2006年4月11日至2006年4月12日。筆者在手術前後於運用歐倫自我照顧（Orem Self Care）模式下，經由觀察、身體評估、傾聽及訪談等方式，收集病人與主要照顧者主、客觀資料，並提供完全代償性、部分代償性、支持與教育性之護理。於手術前期發現病人對手術知識認知錯誤，以支持與教育性護理，協助相關知識之建立；手術中期提供完全代償性護理，因病人髋關節活動受限造成無法正常採膀胱截石臥位，以設計並調整符合病人肢體擺位的姿勢配合手術之進行，而達到預防潛在性的傷害之目標；以傾聽及主動關懷的方式減輕主要照護者等待手術結果的焦慮；在手術後期給予支持與教育性護理，提供病人及主要照顧者預防結石再發的飲食知識，並協助養成良好的飲食行為，同時也促進病人日後自我照顧能力及家庭成員的健康。此個案報告的護理經驗提出，可作為日後照顧類似病人之參考。

This report is to explore an experience of caring for a patient who had ureter stone for lithotripsy and his main caregiver during the patient undergoing operation. The nursing period was from April, 11, 2006 to April, 12, 2006. The author collected subjective and objective data of the patient and his main caregiver through observation, physical assessment, listening and interviews based on Orem nursing system theory. And the author also supplied completely compensatory, partially compensatory, supporting, and educating nursing to them. The care worker helped the client with establishing knowledge related to the operation by supporting and educating nursing when she detected that the client's knowledge was wrong before operation. During operation, designed and modulated patient's position to match up the operation and prevent potential danger by supplying completely compensatory nursing since patient couldn' t be positioned regular lithotomy position that limited by his motion of hip joint. And cared about main caregiver actively and to listen her attentively to relieve her anxiety. To provide patient and main caregiver the knowledge of preventing recurrent ureter stone and to help them with forming behavior of healthy diet promoted patient skill of self-care and family health by supporting and educating nursing after operation. Author hopes to share the nursing experience with other nursing staffs to provide reference of caring for similar patients.
