一位子宮頸癌婦女行子宮根除術的護理經驗

摘要

本文探討一位三十九歲罹患子宮頸癌的婦女，在面臨疾病的威脅、對死亡的恐懼，而接受子宮根除手術後所面對的各種問題之護理經驗；護理期間自2006年7月31日至2006年8月22日，運用「Gordon十一項功能性健康型態」評估，發現個案的護理問題有焦慮、潛在危險性感染、排尿型態改變、潛在性傷害、身體心像紊亂及性行為型態改變。筆者藉由會談、觀察、身體評估等方式收集資料，在護理過程中以同理心與個案建立良好治療性人際關係，透過主動傾聽、關懷、陪伴及鼓勵家屬陪伴及彼此溝通，使個案勇於表達情緒、重建正向的自我概念，協助其接受身體結構、功能的改變，並提供傷口照護、教導日常生活照顧之技能，使身心不適獲得改善，以達完整的照護品質。

A 39-year-old woman with cervical cancer had undergone radical hysterectomy. Goals for her nursing care for the period July 31 to August 22, 2006 included helping her face the diagnosis and the fear of death. Assessment using Gordon's 11-functional health patterns identified her health problems as anxiety, potential risk of infection, change in urinary habits, distorted body image, and change in sexual behavior. The author established a good therapeutic relationship with the patient by conversing with her, examining her, and expressing sympathy. Spending time listening and caring for her, as well as encouraging the family members to do the same, enabled the patient to express her emotions and re-establish a positive self-concept. She was also assisted in accepting the structural and functional changes resulting from the surgery. In addition to wound care, the patient was instructed in performing her own daily care activities. These nursing measures all contributed to the patient's physical and psychological recovery.
