運用Swanson關懷理論照護一位腹膜透析引起腹膜炎合併急性腸阻塞之護理經驗

本文為探討一位因腹膜透析引起腹膜炎合併腸阻塞個案的護理經驗。照護期間為2019年8月23日至9月15日，經觀察、會談、身體評估及病歷查閱收集資料，進行生理、心理、社會及靈性之整體性評估，確立個案有營養狀況少於身體所需、急性疼痛與焦慮之護理問題。透過Swanson關懷理論所提之「了解」、「在旁陪伴」、「為她做某些事」、「使她能夠」與「維持信念」的五大過程，了解個案之想法及需求，鼓勵表達自我感受，陪伴並協助個案面對疾病帶來的衝擊，緩解其焦慮情緒，增強自我照顧能力並給予正向回饋。冀望藉此護理經驗提供同仁照護腹膜炎個案之參考與應用。

This article is to discuss the nursing experience of a peritonitis caused by peritoneal dialysis combined with acute ileus. The care period was from 23 August to 15 September 2019. Observation, interview, physical examination, and medical records were used to collect data and conduct comprehensive evaluation involving physiological, psychological, social, and spiritual aspects. We determined that the patient had nutritional deficiency, acute pain, and anxiety. The five processes of Swanson's Caring Theory (knowing, being with, doing for, enabling, and maintaining belief) were carried out to understand the thoughts and needs of the patient, encourage her to express her feelings, accompany her, assist her in facing the impact caused by disease, alleviate anxiety, increase self-care capabilities, and provide positive feedback. We hope that this nursing experience can provide a reference for nurses caring for similar patients.
