一位血糖控制不良患者合併性功能障礙之護理經驗
Nursing Experience of a Poor Glycemic Controlled Patient Complicated with Sexual Dysfunction
摘要
根據統計約有50%男性被診斷為糖尿病10年後會有中度至重度的勃起功能障礙。本文乃探討一位糖尿病患者因血糖控制不良併發性功能障礙之護理經驗。筆者為糖尿病個案管理師，個案管理期間自民國94年11月14日至民國95年1月22日，於衛教室進行四次訪談，一次約30~40分鐘，並以Gorden十一項功能性健康型態進行資料收集，確立個案之護理問題為性功能障礙、健康維護能力改變、知識缺失等。個案對血糖控制的相關知識不足且無動機，反而對自己性功能方面的問題相當關切。筆者利用個案所關切的性功能障礙問題為切入點，提供相關衛教，並運用衛教單及個別訪談說明控制血糖的重要性，並協助轉介泌尿外科接受評估與治療，終於引發個案主動控制血糖的動機，最後個案不論在藥物、飲食及運動控制均能全力配合，飯前血糖值降至100-130mg/dL，個案最關心的性功能障礙問題已獲得改善並持續規律返診追蹤血糖值，希望此護理經驗能提供護理人員日後在照護此類病患的參考。

Fifty percent of male patients with a 10-or-more-year history of diabetes experience moderate to severe degree of erectile dysfunction. This article presents the nursing care experience with a patient whose poor glycemic control was complicated with sexual dysfunction. The author used the Gordon 11-item functional health scale to assess the patient's health problems including: sexual dysfunction, change of health maintenance ability and lack of knowledge. The patient showed more concern over sexual dysfunction than poor glycemic control. This concern motivated the patient to become receptive to diabetes education. The patient began to show compliance in medication, diet and exercise. The patient's fasting glucose reduced to the normal range of 100-130 mg/dL and consequently improvement in his sexual functions.
