一位低血鉀患者之急診護理經驗
The Nursing Experience of a Patient with Hypokalemic in Emergency
摘要
本篇個案報告旨在描述一位23歲男性患者第三次因心律失常，四肢軟弱無力重返急診求治之護理經驗。護理期間為96年5月6日凌晨兩點四十分至凌晨四時三十分，利用Gordon十一項健康功能型態為評估架構，經由溝通、觀察及會談等方式收集資料，發現個案有：潛在危險性傷害、不遵從藥物及飲食之醫囑、營養狀態改變－含鉀食物少於身體所需等護理問題。急診護理之特色為：個案緊急狀況之處理，即因低血鉀所引起之症狀治療與護理；潛在危險性傷害之預防。經提供安全舒適的治療環境及低血鉀病因、症狀、治療、飲食之有關衛教等介入措施後，個案未發生跌倒且無氯化鉀點滴外漏情形，個案及家屬不僅提升對低血鉀症之病因、症狀及治療的認識，且能說出含鉀食物之種類並願意多攝取含鉀食物及按時服用口服鉀製劑，以避免低血鉀之再發。

This article describes the nursing experience of a 23-year-old male during a one hour and fifty minutes' emergency department visit for arrhythmia and extremity muscle weakness. Based on the Gordon Functional Health Patterns Assessment framework, this patient's health problems included (1) potential risk of injury, (2) non-compliant to medication and dietary regimes, and (3) imbalance in nutritional status due to less-than-body-required dietary potassium. Emergency room nursing focused on treatment, care and injury prevention related to hypokalemia. The patient completed the potassium replacement infusion without complications and received health education about the cause, symptoms, treatment and proper selection of potassium-rich foods. Both patient and family were able to explain the medications and dietary management needed to prevent the recurrence of hypokalemia.
