區域教學醫院的醫師過勞情形之探討

A Study of Burnout in Physician in a Regional Teaching Hospital

摘要


目的:本研究探討醫師的過勞情形,並輔以訪談以了解造成醫師過勞的主因。方法:以嘉義地區某區域教學醫院之醫師為研究對象,發出問卷139份,回收有效問卷99份。以描述性統計、相關分析及變異數分析等統計方法分析。結果:個人疲勞與最近一年最長連續工作時數及目前是否在職進修中達顯著差異;工作疲勞與年齡、最近一年最長連續工作時數及總工作年資達顯著差異;工作過度投入與是否為主管職及目前是否在職進修中達顯著差異;服務對象疲勞與年齡、教育程度、子女數及目前是否持有教職達顯著差異。職場疲勞量表以工作過度投入為最主要疲勞來源。醫師認為工時長、工作量大及值班造成的睡眠不足及壓力會導致醫師過勞,另外也提出自我要求及責任感、健保體制及醫師培訓制度的改變也是造成醫師過勞的主因。結論:重視醫病溝通、補充人力並進行專業分工,以及工時的限制有助於改善醫師過勞現象以促進醫療及生活品質。

Objective: To explore burnout among physicians by questionnaires and interviews. Methods: A total of 139 questionnaires were sent to physicians in a teaching hospital with 99 valid returns. Description statistics, correlation analysis, and analysis of variance were used for analysis. Results: Personal burnout significantly correlated with the longest consecutive work hours in the recent year and the on-job training programs; work burnout significantly correlated with age, the longest consecutive work hours in the recent year and the total working years; over-commitment for workload significantly correlated with holding an administrative position and on-job training programs; burnout by client significantly correlated with age, educational level, number of children, and holding a faculty position. In the burnout inventory, over-commitment for workload is the most important factor. During interviews, physicians believe that sleep deprivation caused by long working hours, heavy workload, and on-call stress would lead to burnout. In addition, high-achieving personality, taking on too many responsibilities, changes in reimbursement policy of National Health Insurance, and adjustment in physician training programs also are main reasons for burnout. Conclusions: Strategies to prevent physician burnout include restricting working time, recruiting more physicians, improving doctor-patient relationship, and providing a teamwork approach to patient care.
