某區域教學醫院尿管相關之泌尿道感染流行病學研究

The Epidemiologic Study of Catheter-Related Urinary Tract Infections in a Regional Teaching Hospital

摘要


本研究旨在探討南部某區域醫院尿管相關之泌尿道感染之病人屬性及醫療因素的分佈情形,收集2007年3月1日至12月31日出院病人共16,672人次中,有尿管置放共1883人次,其中123人次為尿管相關之泌尿道感染,感染率為6.5%,相關研究結果顯示:(1)尿管相關之泌尿道感染與性別無統計上顯著差異;(2)年齡愈大、住院天數過長和尿管置放次數均會造成感染機率增加;(3)疾病診斷方面,感染性疾病最易因尿管置放引發尿路感染;(4)就醫院科別,則發現心臟血管內科病人與尿管相關之泌尿道感染有顯著相關。若進一步控制年齡與性別,則發現神經外科、胸腔內科、心臟內科最易造成院內感染科室。本研究之結果希望能提供醫院臨床實務上參考,並藉此進行醫療及照護上之原因分析,以有效改善照護品質。

This research project studies the character of patients with urinary tract infection caused by urinary catheter and discusses the relevant factors. A total of 16,672 patients hospitalized in a regional teaching hospital in the South Taiwan from March, 1 to December, 31, 2007 were screened, within which, 1883 patients had urinary catheter indwelling and 123 had urinary tract infection (6.5%). The analysis showed that 1) urinary tract infection was not related to sex; 2) age, number of days of hospitalization, and frequency of catheter installed were related to the increase of chances of urinary tract infection; 3) the urinary tract infection were related closely to other kinds of infectious diseases; and 4) the indwelling of urinary catheter in patients in cardiology department were significantly related to higher incidence of urinary tract infection. With age and sex as the control, the first three departments with the highest incidence of urinary tract infection were neurosurgery, respiratory and cardiology. Through this study we wish to provide some evidence for clinical practice, and analyze the character of the patients' distribution and relevant factors to effectively improve the quality of care.
