進階護理人員自評護理能力之探討

Self-Assessment Nursing Competence among Different Levels of Nurses in a Clinical Ladder Program

摘要


本研究採橫斷式調查,目的在瞭解不同層級醫院及不同進階層級護理人員之自評護理能力。採立意取樣,以高屏地區某醫學中心及二家區域醫院,參與專業能力進階制度之所有護理人員爲對象,共1760位,以自擬之個人及工作屬性資料表和自評護理能力量表進行資料收集;自評護理能力量表共41題,測量照護、溝通協調、教學與管理、專業寫作與研究、專業成長等五大能力,共回收1628份問卷(92.5%),共計有效問卷1608份,以SPSS13.0中文版電腦套裝軟體進行資料分析。結果發現:護理人員的自評護理能力在中上程度(3.80±0.54),不同醫院層級及各進階層級的護理人員在自評護理能力具顯著差異,N3(含)以上的護理人員在自評護理能力及五大能力均表現最佳;醫學中心護理人員在照護能力、溝通協調能力、教學與管理能力與區域醫院護理人員具顯著差異,本結果有助於護理主管輔導護理人員完成各級進階要求及提升護理能力之參考。

The study conducted a cross-sectional survey. The aim was to understand the degree of self-assessment nursing competence among nurses on different clinical ladders and two levels of hospitals. Using purposive sampling, this study recruited 1760 nurses who participated in clinical ladder programs of a medical center and two regional hospitals in the Kao-Ping area. Data were collected using a personal and working information sheet and self-assessment nursing competence scale. The nursing competence scale comprised 41 items measuring five competencies: care delivery, communication and coordination, teaching and management, writing and research, and professional development. Of 1,628 returned questionnaires, 1,608 questionnaires were completed (92.5%). Data were analyzed with SPSS13.0. Research results indicated moderate self-assessment of nursing competency (3.80±0.54). Significant differences in self-assessed nursing competency were observed among nurses on different clinical ladders and between the two levels of hospitals. Nurses ranked N3 and above performed best in selfassessment of nursing competence and the five nursing competencies. Degrees of nursing competence in the dimensions of care delivery, communication and coordination, and teaching and management between nurses from the medical center and those from the regional hospitals. These results provide a reference for nursing administrators attempt to assist nurses in fulfilling the requirements of clinical ladders and improving their nursing competency.
