利用大腸直腸癌基因晶片偵測循環癌細胞以進行大腸直腸癌術後預後的追蹤評估

摘要


目的:基因晶片的技術已經廣泛地運用在各種不同的癌症。分子檢測技術的確可以助於臨床上得知手術同時癌細胞有無根除以及術後的輔助治療的策略擬訂。本研究目的主要是分析及評估以基因晶片,偵測大腸癌患者在術前及術後的hTERT、CK-19、Ck-20、CEA、Her2/neu、hnRNP、C-Met、MUC1、CRC1-40各種基因表現及其臨床價值。方法:本研究研究對象為20位接受手術治療的大腸癌患者,收集其手術前腫瘤組織、手術前後病人的周邊血液,以基因晶片技術偵測其術前及術後癌細胞指標分子的mRNA基因表現並分析其臨床意義。結果:hTERT、Ckl19、Ck20、CEA、cMet、MUC-1、CRC-1、CRC-2在大多數的病人術前皆呈現較高的基因表現且在術後下降。其中以CEA在術前術後下降幅度最為顯著。CRC-3及CRC-4在所有病人手術前後都偵測不到。至於在手術前後的表現對淋巴結轉移,腫瘤大小及腫瘤分期都則沒有顯著的關聯性。結論:基因晶片可偵測大部份的大腸癌患者之周邊血液之許多癌細胞指標分子並發現其在手術後之下降,這些分子的表現具有術後追蹤體內殘餘癌細胞活動之價值與潛力,值得進一步的研究。

Objectives. Gene chip has been developed for the evaluation of human cancer. Techniques in molecular detection can be uti1ized to identify possible circulating or residual tumor cells after operation and can guide further treatment strategies. The study is designed to assess the value of gene chips in evaluation of tumor markers mRNA in patients with colorectal cancer before and after surgery. Methods. We collect the peripheral blood of 20 colorectal cancer patients before and after surgery and analyze the mRNA molecular markers of hTERT, CK-19, CK-20, CEA, and other tumor markers for the detection of circulating tumor cells. The c1innicopathological significance will also be evaluated. Results. Among the tumor markers detected, hTERT、CK19、CK20、CEA、cMet、MUC-1、CRC-1、CRC-2 were found to be expressed in elevated levels before surgery and decreased in amount after operation. The magnitude of decrease was most prominent in CEA. CRC-3 and CRC-4 can not be detected in all 20 patients. The association of tumor marker expression with c1inicopathological parameters was not significant. Conclusions. Gene chips can be used to detect many tumor markers, whose decrease in expression can also be found after operation, from circulating cancer cells in patients with colorectal cancer. These molecules potentially serve as surrogate of activities of residual cancer cells after operation and deserve further study.
