Posteromedial dislocation of elbow with lateral condyle fracture is a rare disease entity and only few cases have been reported. These study reports six children who had posteromedial dislocation of elbow with lateral condylar fracture of distal humerus: three cases received close reduction of elbow dislocation and open anatomic reduction of lateral condyle with pinning fixation had excellent result, in which the fractured lateral condyle adhered to the radial head through the intact posterior capsule and lateral collateral ligament was found. Two cases received unanatomic reduction which had poor result, another misdiagnosis case which had delayed surgery two months later had poor result; the poor outcomes included the malunion of the fracture, heterotophic bone formation, elbow stiffness and finally cubitus vara. Open anatomic reduction of lateral condyle is advised.
