危險值通報系統在病人安全防治之規劃與檢討－以衛生福利部某區域醫院為例

摘要

檢驗檢查報告之危險值及重要異常結果之及時通知，在臨床照護團隊的相關決策及病患的後續醫療處置上扮演相當重要的角色，完善的危險值通報系統的建立不但可以有效降低病人延遲治療的風險、大幅減少人力工作負荷及錯誤率，且能確保結果與訊息之正確傳遞、促進醫療人員間的溝通，提升醫療有效性。本篇主要是參考現金各醫院通報系統的設置與規劃，並以SWOT方法分析系統之優劣，以便對衛福部醫院系統提供改進與建置的方向；建立衛福部醫院危險值通報系統的完整傳送與接收機制。以SWOT分析方法綜合各醫院資料，完善的危險值通報系統能有效發揮緊急檢驗及檢查的功能，使相關人員能將臨床訊息迅速運用於病人後續的醫療處置，增加病人治療的即時性及有效性，落實病人安全目標，提升醫療照護品質。參考以上各醫院危險值通報系統的建置方向與使用經驗，完善的通報系統不但能大幅減少人力工作負荷、減少錯誤率且能確保檢查等危急值或其他重要結果之及時通知與訊息之正確傳遞；加強各單位之多方溝通模式，使醫療團隊中的每一成員都充分掌握即時訊息，增進醫療處置效率，達成以病人為核心的醫療照護並保障及促進病人安全。

The timely notification of a patient's abnormal laboratory results is very important to his/her physician, especially when those results indicate life-threatening status. An automatic crucial lab data notification system would not only reduce the need for a staff member to communicate this information and ensure the accuracy of information transferred it would also, avoid unnecessary delays in transferring that information. This article compares the automatic crucial lab data notification systems in many hospitals using SWOT analysis. It offers recommendations on how to improve the efficiency of automatic crucial lab data notification system in hospitals operating under the Ministry of Health and Welfare.
